Dear Sir,
Due to severe financial crisis I was unable to settle the dues against Credit Card No: [insert number]. After discussion with your department’s representatives it was mutually decided that I would be relieved of the liabilities upon payment of [insert amount in numeric] (insert amount in words inside this parenthesis) only.
However, since I am not financially stable, it would be highly appreciated if I am given the opportunity to pay off the above amount in 3 (three) installments as per the following schedule:
